What you will do

• Conduct conversations about goals, values, and advance care planning via phone, email, and video.
• Perform comprehensive symptom assessments and provide longitudinal support for members with serious illness.
• Facilitate end-of-life care discussions and manage urgent clinical escalations.
• Collaborate with Nurse leaders and Medical Directors to ensure alignment with clinical protocols.
• Coordinate with payers and providers to optimize care delivery and minimize readmissions.
• Partner with non-clinical team members to address social determinants of health, such as food and transportation.

Requirements

• Must be located within the lower 48 United States.
• Active, unrestricted compact Registered Nurse (RN) license (NY, IL, or MI licensure preferred).
• Degree in Nursing (ADN or BSN required; BSN preferred).
• At least 5 years of nursing experience, including 3 years in outpatient palliative care and/or home hospice oncology.
• OCN certification required at hire or a commitment to obtain it within 2 years.
• Ability to work shifts from 11:30 - 8 EST and provide occasional Saturday coverage.

Nice to have

• Certified Hospice and Palliative Nurse (CHPN) or other oncology certifications (AOCN, AOCNS, CCM).
• Fluency in Spanish, both written and verbal.
