Location: Must be based in South Texas, USA (Remote with frequent travel required).

Salary: $23–$28 per hour.

What you will do

  • Serve as a community-based resource, building direct relationships with patients and caregivers through in-person engagement.
  • Provide hands-on support for program re-enrollment, onboarding, and mobile app utilization.
  • Conduct telephonic outreach to re-engage patients who have missed onboarding or service utilization.
  • Educate families on available services and connect them to local community resources and social support systems.
  • Collaborate with behavioral health teams and facility staff to support discharge planning and remove barriers to care.
  • Support non-clinical care coordination, including pharmacy access, primary care connections, and addressing social determinants of health.

Requirements

  • Must be based in South Texas and able to travel frequently throughout the area.
  • Bilingual proficiency in Spanish is required.
  • National CHW certification.
  • Bachelor’s degree.
  • 3+ years of experience in a patient-facing healthcare setting.
  • Valid driver’s license.
