What you will do

• Conduct comprehensive in-home, video, and telephonic clinical assessments for members with chronic conditions.
• Order and interpret diagnostic studies to institute appropriate therapy within scope of practice.
• Manage medications and treatments for palliative care and symptom alleviation.
• Facilitate advance care planning and end-of-life discussions with patients and families.
• Coordinate care with interdisciplinary teams and update patient care plans.
• Ensure accurate EMR documentation to support medical coding and risk adjustment methodologies.

Requirements

• Graduate from an accredited PA program or Nursing Approved Master’s/Post Master’s program.
• 3 years of clinical experience as an NP or PA in primary care, geriatrics, or home health.
• Current Physician Assistant License or PA RN licensure with CRNP and Prescriptive Authority.
• Valid driver's license and reliable transportation for frequent travel to patient homes.
• Ability to work autonomously in a community-based setting.
• Proficiency with EMR systems and mobile technology.

Nice to have

• Board certification in Family Medicine, Internal Medicine, Geriatrics, or Palliative Care.
• Experience conducting in-home assessments or house calls.
• Knowledge of CMS regulations and population health management.
• DEA controlled substances license.
