What you will do

  • Lead clinical integration efforts for newly acquired agencies to ensure regulatory compliance.
  • Develop and implement transition plans, including staff orientation and competency validation.
  • Collaborate with executive leadership to assess clinical infrastructure and identify operational risks.
  • Manage project timelines, stakeholder communication, and progress tracking for integration initiatives.
  • Implement standardized workflows to enhance efficiency and quality across all agencies.
  • Provide guidance to agency leaders during post-acquisition stabilization.

Requirements

  • Must be a Licensed Registered Nurse or hold a related licensed discipline (PT/OT/ST).
  • Minimum of five years of Home Health and/or Hospice experience.
  • Previous experience as an agency leader and with mergers and acquisitions.
  • Strong understanding of legal and regulatory frameworks governing post-acute care.
  • Ability to manage complex projects and work independently.
  • Excellent verbal and written communication skills.
