Salary: Physicians $240,000–$300,000; NPs $145,000–$165,000

What you will do

• Provide palliative care in patients' homes, managing pain, symptoms, and offering emotional/spiritual support.
• Conduct assessments for serious conditions, develop personalized care plans, and manage medical needs.
• Collaborate with patients, families, and multidisciplinary teams to align care with goals and wishes.
• Participate in shared on-call rotation as directed.

Requirements

• For Physicians: MD/DO, Board Certified/Eligible in Palliative Care or related; 3+ years experience; licensed (or willing) in New Jersey/Georgia with DEA/CDS.
• For NPs: Based in New Jersey/Georgia; BSN + Master's; active NP license, DEA, CDS in New Jersey; inpatient palliative/hospice experience.
• Experience in home-based or value-based care preferred.
