Salary: $140,000 - $165,000 USD

What you will do

• Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
• Perform post-discharge follow-up visits to reduce avoidable hospital readmissions.
• Collaborate with interdisciplinary teams using the Clover Assistant clinical insight platform.
• Deliver proactive, personalized primary and supportive care directly in members' homes.

Requirements

• Active and unencumbered GA Registered Nurse license.
• Certified Advanced Practice Nurse Practitioner (APN).
• Master’s degree or post-master’s certificate in advanced clinical practice.
• Minimum of 1 year of NP experience in a clinical setting.
• Active DEA registration and current Basic Life Support (BLS) certification.
• Located in or near Atlanta, Georgia with the ability to travel locally for home visits.
