Salary: $140,000 - $165,000 USD

What you will do

• Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
• Perform post-discharge follow-up visits to reduce hospital readmissions.
• Collaborate with interdisciplinary teams using Clover Assistant and other data-driven clinical tools.
• Provide primary and supportive care directly in members' homes to improve health outcomes and quality of life.

Requirements

• Located in or near Gloucester County, New Jersey with ability to travel locally for home visits
• Active and unencumbered NJ Registered Nurse license
• Certified Advanced Practice Nurse Practitioner (APN)
• Master’s degree or post-master’s certificate in advanced clinical practice
• Minimum 1 year of NP experience in a clinical setting
• Active DEA and CDS registration
• Current Basic Life Support (BLS) certification

Nice to have

• Experience in home care or house call settings
