Nurse Practitioner (Medtech)

Location: Must be based in or near Albany, Georgia and able to travel locally for home visits

Salary: $140,000 - $165,000 USD

What you will do

• Conduct comprehensive in-home health assessments for enrolled Medicare Advantage members.
• Perform post-discharge follow-up visits to reduce hospital readmissions.
• Collaborate with interdisciplinary teams using the Clover Assistant data-driven clinical tool.
• Deliver primary and supportive care directly in the homes of vulnerable members to improve health outcomes.

Requirements

• Active and unencumbered GA Registered Nurse license
• Certified Advanced Practice Nurse Practitioner (APN)
• Master’s degree or post-master’s certificate in advanced clinical practice
• Minimum of 1 year of NP experience in a clinical setting
• Active DEA and CDS registration
• Current Basic Life Support (BLS) certification
