What you will do

• Act as the primary liaison and subject matter expert for nephrology practices.
• Facilitate the transition to Value-Based Care, including CMS Kidney Care Contracting and Payor Agreements.
• Analyze data to uncover insights and identify improvement opportunities for partners.
• Develop and deliver training and onboarding for Network Providers.
• Execute company initiatives and maintain accurate documentation in the CRM.
• Communicate complex program objectives to internal and external stakeholders.

Requirements

• Bachelor’s degree (Advanced Degree preferred).
• Proven experience working with physicians and medical practices.
• Must be based in the United States.
• Ability to travel up to 50% of the time to assigned regions.
• Strong communication and interpersonal skills for stakeholder collaboration.
• Proactive approach to continuous learning and attention to detail.
