TL;DR

RN Case Manager (Healthcare): Lead coordination of multidisciplinary teams to deliver holistic, person-centric care management for diverse health plan members with an accent on assessing needs, developing care plans, and optimizing health across the care continuum. Focus on engaging members, arranging resources, coordinating care delivery, advocating for patients, and monitoring plan effectiveness.

Location: Remote however must be local to Michigan (statewide in MI)

Pay Rate: up to $40/HR

What you will do

  • Lead coordination of regionally aligned multidisciplinary teams for holistic telephonic/digital care.
  • Assess member health, psychosocial needs, cultural preferences, and support systems.
  • Develop individualized care plans, address barriers, identify gaps, and promote health outcomes.
  • Arrange community resources, mental health, substance abuse, financial, and disease-specific services.
  • Coordinate care among providers, agencies, and family; advocate for members.
  • Deliver education on health literacy, self-management, medications, and nutrition.
  • Monitor, evaluate, and adjust care plans; document interactions accurately.
  • Prepare for discharges/transfers, secure equipment/transportation, and educate on post-transition care.

Requirements

  • Current, active, unrestricted Michigan Registered Nurse license required
  • Nursing Diploma or Associate's degree in nursing required; Bachelor's degree strongly preferred
  • 3+ years clinical nursing experience in clinical, acute/post-acute, and community settings
  • 1+ year case management experience in managed care setting strongly preferred
  • Certification in Case Management (CCM) required or obtained within 18 months
  • Experience managing patients telephonically and via digital channels preferred

Nice to have

  • Certification in Chronic Care Professional (CCP)
