What you will do

  • Engage with newly discharged patients telephonically to conduct assessments and identify clinical needs.
  • Develop and evaluate comprehensive care plans in partnership with patients and their caregivers.
  • Educate patients on chronic conditions, including identifying "red flags" and improving medication adherence.
  • Coordinate care and handoffs with Nurse Practitioners, Physicians, and Community Guides.
  • Collaborate with PCP partners to deliver evidence-based and accessible healthcare solutions.

Requirements

  • Unrestricted Compact RN license obtained in the United States.
  • 3-4 years of experience in RN Care Management or telephonic case management.
  • Must be based in the US and located in CT, MT, or PT time zones.
  • Ability to secure additional state licensure as required.

Nice to have

  • Experience with Medicare Advantage or health insurance.
  • Prior experience in Special Needs Plan, Population Health, or Transitions of Care case management.
  • Comfort working in an environment guided by performance and productivity metrics.
