What you will do

  • Engage with newly discharged patients telephonically to identify immediate needs and risk factors.
  • Conduct clinical assessments to identify comorbidities, medications, psychosocial factors, and patient preferences.
  • Develop and continuously evaluate personalized care plans including goals and interventions.
  • Educate patients on chronic conditions, teaching them to identify red flags and manage exacerbations.
  • Partner with Nurse Practitioners, Physicians, and Community Guides to ensure seamless post-discharge handoffs.
  • Collaborate with PCP partners to deliver evidence-based and accessible healthcare.

Requirements

  • Unrestricted Compact RN license obtained in the United States.
  • 3-4 years of experience in RN Care Management or telephonic case management.
  • Fully bilingual in English and Spanish.
  • Must be located in CT, MT, or PT time zones.

Nice to have

  • Experience with Medicare Advantage.
  • Prior experience in Special Needs Plans or Population Health.
  • Comfort working in an environment guided by performance and productivity metrics.
