What you will do
- Conduct telephonic and video visits to establish trust and build strong relationships with patients and caregivers.
- Perform psychosocial assessments and create personalized care plans to address socioeconomic and psychosocial barriers.
- Facilitate goals of care and advanced care planning discussions, including hospice care transitions.
- Collaborate with an interdisciplinary team of clinicians, nurses, and coordinators to ensure patient-centered care.
- Coordinate long-term care planning, placements, and behavioral health referrals.
- Document all clinical interactions accurately within the electronic health record.
Requirements
- Master’s degree in social work and an active license.
- Must be based in the USA.
- Minimum of 2 years expertise in palliative or hospice care.
- Proficiency in leading goals of care conversations and supporting people through grief, loss, and bereavement.
- Willingness to obtain a certified hospice and palliative social work credential within one year of employment.
- Comfort with technology and ability to succeed in a fast-paced, work-from-home environment.
Nice to have
- Bilingual proficiency in English and Creole, Spanish, or Vietnamese.
- Experience in health insurance, specifically Medicare Advantage.
- Experience in geriatrics.
