What you will do

• Conduct telephonic and video visits to establish trust and build strong relationships with patients and caregivers.
• Perform psychosocial assessments and create personalized care plans to address socioeconomic and psychosocial barriers.
• Facilitate goals of care and advanced care planning discussions, including hospice care transitions.
• Collaborate with an interdisciplinary team of clinicians, nurses, and coordinators to ensure patient-centered care.
• Coordinate long-term care planning, placements, and behavioral health referrals.
• Document all clinical interactions accurately within the electronic health record.

Requirements

• Master’s degree in social work and an active license.
• Must be based in the USA.
• Minimum of 2 years expertise in palliative or hospice care.
• Proficiency in leading goals of care conversations and supporting people through grief, loss, and bereavement.
• Willingness to obtain a certified hospice and palliative social work credential within one year of employment.
• Comfort with technology and ability to succeed in a fast-paced, work-from-home environment.

Nice to have

• Bilingual proficiency in English and Creole, Spanish, or Vietnamese.
• Experience in health insurance, specifically Medicare Advantage.
• Experience in geriatrics.
