TL;DR

Consultant – Clinical Performance Improvement (Care Transitions) – Healthcare: Analyzing organizational performance against industry standards, leading benchmarking studies, and developing project plans for clinical quality and operational efficiency improvements with an accent on care transitions, throughput, and patient placement. Focus on implementing evidence-based practices, optimizing discharge processes, and enhancing post-acute care coordination to reduce complications and improve outcomes.

Location: Remote - USA. Candidate must be able to submit verification of legal right to work in the U.S., without company sponsorship. Willing to travel consistently (50% - 75%).

Salary Range: $90,000 - $135,000 per year

What you will do

  • Support project workstreams under SME direction or act as SME in core knowledge areas like care transitions.
  • Develop and implement project plans, client deliverables, and future-state processes for performance improvement.
  • Lead work teams, meet independently with clients and executives, and create client-facing materials like slide decks.
  • Monitor implemented workstreams, provide feedback, and establish client processes to maximize quality and efficiency.
  • Contribute to firm advancement through work teams and utilize non-productive time for professional development.

Requirements

  • 3+ years in clinical performance improvement consulting or 5-10 years industry experience in relevant role with licensure (e.g., Registered Nurse, Social Worker) and 2+ years leadership.
  • Bachelor's Degree in health or related field required; Master's preferred (must enroll and complete within 3 years if not held).
  • Proficient in Microsoft Excel, PowerPoint, Word; experience with Tableau preferred.
  • Strong analytical, time management, and resource management skills.
  • Operational knowledge of care management, status assignment, patient placement, throughput, discharge barriers, and post-acute care.
  • Willing to travel 50%-75% depending on project requirements.

Nice to have

  • Lean Six Sigma Black Belt Certification or PMP.
  • Understanding of labor productivity, benchmarking, and clinical coding/documentation.
