TL;DR

Location: Remote (Must be based in the US, Central Time Zone alignment)

Salary: $70,000 - $77,000 + annual bonus

What you will do

  • Conduct biopsychosocial assessments to address behavioral, social, and emotional needs of patients and families.
  • Develop and modify individualized care plans for high-risk pediatric patients.
  • Provide crisis intervention to de-escalate and stabilize patients and family members.
  • Coordinate care with external providers, school systems, and social services agencies.
  • Offer psychoeducation on mental health diagnoses, treatment adherence, and progression.
  • Collaborate within an interdisciplinary team of pediatricians and nurses to monitor patient progress.

Requirements

  • Master's degree in Social Work or a related field (Required)
  • 3-5 years of post-graduate experience in behavioral health case management (Required)
  • Experience working with pediatric populations and family systems (Required)
  • Proficiency in motivational interviewing and telehealth service delivery.
  • Ability to work afternoons/evenings and travel up to 10% for training (Required)
  • Bilingual Spanish skills are preferred

  • Provisional licenses (LMSW, PLPC, LAMFT).
