Bilingual (Spanish) Advanced Practice Provider (NP or PA): Palliative Care (Medical)

Location: Remote (USA). Must hold an unrestricted advanced practice license in one or more of the following states: FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, PA

Salary: $130,000 – $170,000 (Total Compensation)

What you will do

• Perform initial assessments and develop Palliative Plans of Care for members newly enrolling in the program.
• Manage a longitudinal panel of 40-50 high-risk patients via regular telephonic and video encounters.
• Provide consultative visits for advanced symptom management, complex goals of care, and hospice transitions.
• Evaluate hospital admissions to identify root causes and develop solutions to maintain low readmission rates.
• Collaborate with interdisciplinary teams through regular huddles and weekly interprofessional meetings.
• Provide on-call weekend and holiday coverage on a rotating basis.

Requirements

• Unrestricted advanced practice license in one or more of the listed US states (FL, TX, AZ, OH, CO, OR, TN, IL, AL, HI, NC, SC, PA).
• 5 years of experience in advanced practice, with at least 3 years specializing in Palliative care.
• ACHPN® credential or the willingness to obtain it within the first six months of employment.
• Bilingual proficiency in Spanish.
• Ability to flex between time zones and succeed in a fast-paced, work-from-home environment.
• Comfort with technology, including EHRs, Google Office, and Slack.

Nice to have

• ELNEC® or EPEC® training.
• Bilingual skills in Creole or Vietnamese.
• Prior experience in ICU, oncology, geriatrics, or home-based palliative care.
• Experience in program development or interprofessional team leadership.
