Focus on building longitudinal relationships, population health initiatives, co-managing with partners, and reducing hospital readmissions.

Location: Fully remote nationwide (US). Must be licensed or eligible in US states (e.g., MA, NJ, NY, MN, WI, IA, SC, NC, OR, VA, TX); IMLC preferred. Support provided for up-licensing.

Compensation: $255,000 - $290,000 USD

What you will do

• Provide high-quality preventative, acute, and chronic care via virtual visits.
• Manage a dedicated panel of patients as their primary care physician.
• Co-manage patients with assigned physicians and external partners.
• Educate patients, prescribe medications, handle follow-ups, and refer to specialists.
• Work on population health initiatives like Annual Wellness Visits and post-discharge support.
• Collaborate on clinical guidelines and perform self-evaluation for best practices.

Requirements

• Board-Certified in Family Medicine
• 3+ years post-residency experience; primary care and behavioral health; acute/urgent care.
• Experience with virtual visits/telemedicine required.
• EPIC EMR experience; currently enrolled or eligible as Medicare provider.
• Clean background and malpractice history; commit to 40 hours/week (36 clinical), including evenings/weekends.
• Handle high patient volume (3+ per hour); tech-savvy and organized.

Nice to have

• Licensed in MA, NJ, NY, MN, WI, IA, SC, NC, OR, VA, TX or active IMLC.
• 3+ years in-person primary care.
• Spanish-speaking ability.
