Location: Remote (Must be based in the US and hold an active OR state medical license)

Compensation: $230,000 - $290,000 USD

What you will do

• Provide remote preventative, acute, and chronic care via video visits.
• Manage a dedicated panel of patients as their primary physician.
• Coordinate specialist referrals and acute care services.
• Implement population health initiatives including AWVs and care gap closures.
• Support patients post-discharge to reduce hospital readmissions.
• Adhere to clinical guidelines through continuous self-evaluation.

Requirements

• Board Certification in Family or Internal Medicine.
• Active Oregon (OR) state medical license.
• 3+ years of post-residency experience.
• Minimum 2+ years in an in-person primary care setting.
• Prior experience with EPIC EMR and virtual visits.
• Ability to work 40 hours per week (including Mondays and Fridays).
