What you will do

• Collaborate with physicians and dialysis staff to complete and document dialysis visits and address medical issues.
• Provide chronic kidney disease care in office settings and coordinate comprehensive treatment plans.
• Support care transitions, including annual physicals and post-hospital discharge planning.
• Deliver care across multiple settings, including rehab centers, skilled nursing facilities (SNF), and office environments.
• Perform diagnostic evaluations and prescribe or recommend appropriate therapies.

Requirements

• Master’s degree in nursing or related field with 4-10 years of equivalent experience.
• Current Arizona RN license and Nurse Practitioner or Physician Assistant license.
• Proficiency in Microsoft Office and experience with ICD-9 and CPT coding.
• Solid interpersonal skills and a flexible schedule.
• Reliable transportation for travel between sites.
