What you will do

• Deliver telemedicine patient care for obesity and diabetes, including insulin titration and advanced diabetes technology like CGMs and pumps.
• Collaborate with endocrinologists and dietitians to co-manage shared patient panels and review charts for continuity.
• Provide advanced treatment planning for complex endocrinology cases, prescribe non-controlled medications, and handle escalated concerns.
• Conduct weekly collaborative meetings and asynchronous communication with physicians for case reviews.
• Ensure adherence to clinical documentation, protocols, and guidelines for complex diabetic populations.
• Support team development through feedback, education, and participation in quality improvement initiatives.

Requirements

• At least 2 years practicing advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens; able to sign collaboration agreement with Endocrinologist.
• Demonstrated competency in managing complex comorbidities like CKD, ESRD, and CAD/CV conditions.
• Experience in telemedicine care with strong communication and interpersonal skills.
• Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity.
• Commitment to patient-centered, culturally competent care.

Nice to have

• OMA certification (CAEOM) or clinical experience in obesity medicine including oral and injectable medications.
