What you will do

• Collaborate with physicians and dialysis staff to complete and document dialysis visits and address medical issues.
• Provide chronic kidney disease care in office settings and coordinate comprehensive treatment plans.
• Support care transitions, including conducting annual physicals and managing post-hospital discharge planning.
• Deliver diagnostic evaluations and treatment planning across settings, including rehab, SNF, and office environments.
• Prescribe and recommend appropriate therapies while maintaining professional conduct.

Requirements

• Master’s degree in nursing or a related field.
• Current Arizona RN license and Nurse Practitioner or Physician Assistant license.
• 4-10 years of equivalent education and experience.
• Proficiency in Microsoft Office and experience with ICD-9 and CPT coding.
• Reliable transportation for travel between sites in the West Valley area.
