What you will do

• Maintain accurate, timely, organized, and confidential medical records in compliance with HIPAA.
• Obtain patient information from healthcare professionals and enter it into electronic medical record systems for Chronic Care Management programs.
• Collect, organize, and review patients’ past medical histories to support safe and personalized care.
• Identify contraindications to exercise and participation in cardiac or pulmonary rehabilitation programs using clinical protocols.
• Apply knowledge of cardiac and pulmonary medications to support medication reconciliation and identify risks.
• Communicate with patients, physicians’ offices, founders, and the clinical team while providing administrative support and improving digital care delivery.

Requirements

• Bachelor’s degree in Clinical Exercise Physiology or Exercise Science.
• At least two years of experience in a medical office setting, preferably in cardiac or pulmonary rehabilitation.
• Proficiency with medical records systems and software, medical terminology, and medical record documentation.
• Excellent attention to detail, accuracy in data entry, organization, problem-solving, and communication skills.
• Familiarity with HIPAA regulations and commitment to patient privacy and confidentiality.
• Must work remotely from within the United States.

Nice to have

• At least two years of experience in a similar role.
• Remote work experience.
• ACSM Clinical Exercise Physiologist Certification.
• AACVPR CCRP certification.
• Registered Health Information Technician certification or a similar credential.
