Salary: $27-$30/hour

What you will do

• Screen members for Hypertension Clinic eligibility and provision BP monitors while teaching proper usage
• Conduct hypertension assessments reviewing medical history, medications, adherence, and lifestyle
• Deliver intensive counseling on nutrition, exercise, smoking cessation, and barrier overcoming
• Coach members using motivational interviewing to achieve sustainable behavioral changes
• Coordinate care with NPs, pharmacists, social workers, and PCPs as primary point of contact
• Maintain documentation, schedule follow-ups, conduct outreach, and close quality measure gaps

Requirements

• Location: Must be in USA time zones (EST/CST/MST/PST)
• Experience as Medical Assistant or in clinical/healthcare setting
• At least 2 years in longitudinal chronic disease management (primary/specialty care)

Nice to have

• Spanish bilingual
• Experience with Medicare patients
