Salary: $78,000 - $90,000

What you will do

• Manage a caseload of members with behavioral health needs, providing relationship-based outreach and navigation.
• Coordinate access to psychotherapy and psychiatry through referrals and warm handoffs.
• Assess Social Determinants of Health (SDOH) and connect members to appropriate community resources and programs.
• Collaborate with a multidisciplinary care team to develop and implement individualized care plans.
• Provide brief, skills-based behavioral health support and crisis de-escalation within protocol.
• Represent the organization at community events to build trust and support member activation.

Requirements

• Active, unrestricted Clinical Social Work License (LCSW)
• Fluency in English and Spanish (verbal and written)
• Must be authorized to work in the United States
• Master’s degree in Social Work
• 1–3 years of experience in healthcare or community navigation

Nice to have

• Knowledge of Latino customs and cultural norms
• Experience supporting members with SMI/SUD needs
• Training in motivational interviewing
• Familiarity with Google Workspace and Electronic Health Records (EHR)
