What you will do

• Conduct virtual assessments and develop individualized treatment plans for diabetes patients, including medication management and lifestyle modifications.
• Collaborate with an interprofessional team of PCPs, endocrinologists, pharmacists, and social workers to ensure continuity of care.
• Serve as a clinical advisor and provide escalation support for specialty clinic staff during business hours.
• Perform comprehensive disease reviews and assess for quality of care (STARS/HEDIS) interventions.
• Utilize and provide feedback on a proprietary electronic health information system to improve the user interface.
• Maintain accurate patient records in compliance with legal, ethical, and telehealth regulations.

Requirements

• Must be based in the USA.
• APRN with 5+ years of outpatient clinical practice, ideally in endocrinology or diabetes focused clinics.
• Minimum 2 years of experience specifically concentrated in diabetes management and education.
• Multi-state licensure is required, with a willingness to obtain additional licenses as requested.
• Master's or Doctoral degree in Nursing with a specialization in diabetes care.
• Active and clear RN and APRN licenses, APRN certification, and BLS certification.

Nice to have

• Experience in virtual care and a strong desire to perform virtual visits.
• Understanding of managed care, including STARS/HEDIS measures and clinical care gaps.
• Proficiency in Spanish.
