Outcomes-Based Care Manager (AI)

Location: Remote - US

Base salary: $130,000–$150,000 USD per year

What you will do

• Advise clinical directors, operations leaders, and executives on adopting outcomes-based care models.
• Lead outcomes-focused onboarding, implementation, optimization engagements, discovery sessions, workshops, and recurring business reviews.
• Assess customer data practices and outcomes maturity, build improvement roadmaps, and train clinical teams on outcomes measurement.
• Apply standardized outcome measures and benchmarks across ABA and IDD services; review progress trends, identify at-risk caseloads, and recommend data-informed care adjustments.
• Help configure clinically sound reports and dashboards and prepare outcomes narratives for payers, funders, and families.
• Coach responsible use of AI-enabled tools, share field insights with Product, Clinical, and Data teams, and support reusable playbooks, pre-sales, renewals, webinars, and conferences.

Requirements

• Active BCBA certification in good standing or an equivalent clinical credential in autism/IDD care.
• 4+ years of clinical experience in ABA or IDD services, including supervisory, clinical leadership, or quality-focused work.
• Experience advising, training, or consulting with external clients or stakeholders.
• Strong understanding of clinical data collection, progress monitoring, and translating treatment data into outcomes.
• Ability to interpret data and communicate findings clearly to clinical and executive audiences.
• Comfort with software platforms and data tools, curiosity about applying AI to clinical and operational work, and ability to manage multiple customer engagements independently.
• Clinical credibility, sound ethical judgment, analytical thinking, data fluency, clear written and verbal communication, executive presence, collaboration, adaptability, and a continuous-improvement mindset.

Nice to have

• Experience with value-based care, outcomes-based contracting, or payer quality programs.
• Familiarity with hirify.global or comparable practice management and EHR platforms.
• Background in program evaluation, quality improvement, or clinical analytics.
• Experience in a customer-facing role at a software or health tech company.
• Experience presenting at conferences or to executive audiences.
