Practice Transformation Specialist (Florida Panhandle)

Location: Remote, but candidates must be residents of Florida and travel in person to practices in Fort Walton Beach, Destin, and Pensacola approximately once per month per practice; schedules may vary according to practice needs.

Salary: $65,000–$75,000 per year base, plus bonuses and equity.

What you will do

• Implement products and execute ACO initiatives under the direction of Market Leadership.
• Support wellness visits, transitions of care, active management, and practice transformation plans.
• Train office staff, physicians, and providers on processes and technologies in coordination with the value-based care curriculum.
• Assess ongoing training needs and build lasting relationships with physicians, providers, and healthcare system executives.
• Support practice financial and quality performance across relevant metrics.
• Listen to practice concerns and communicate improvement opportunities to managers and leads.

Requirements

• Must be a resident of Florida.
• Graduate of an accredited college or university, or 4–6 years of significant and relevant medical practice management experience in lieu of the education requirement, particularly significant administrative experience in a clinic setting.
• 3 years of experience using Electronic Health Records for clinical or practice management processes.
• Excellent oral and written communication skills, including public presentation skills.
• Organizational, project management, and team-building skills.
• Familiarity with process mapping and workflow analysis tools.
• Excellent computer skills and willingness to learn additional software applications.
• Ability to travel in person to Florida practices for regular on-site check-ins, typically once per month per practice, with frequency flexing according to practice needs.
