Case Manager (Healthcare)

Location: Remote within the United States, serving La Crosse County, Wisconsin and surrounding areas; occasional travel to offices and member residences is required.

Salary: $53,700–$72,600 per year. A $3,000 hiring bonus is also offered, paid in two installments.

What you will do

• Assess members’ health, safety, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services in the least restrictive environment.
• Conduct face-to-face assessments at enrollment and at least every six months, typically at members’ residences.
• Conduct quarterly in-person visits and maintain monthly telephone contact.
• Arrange social integration, community, employment, housing, and other non-medical support services.
• Evaluate risks, provide member education, manage service authorizations, and maintain case documentation.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits.
• Ability to travel up to 40% and conduct recurring in-person visits across the assigned service area.
• Availability Monday through Friday, 8:00 a.m. to 4:30 p.m. CST.
• Ability to complete required TB screening for this patient-facing role.

• Residence within 45 minutes of the assigned coverage area.
• Case management experience and electronic case-note documentation experience.
• Knowledge of community health, social service agencies, and local resources.
