Senior Manager, Care Management - Transitions of Care

Salary: $110,000–$173,000 annually

What you will do

• Lead a frontline workforce of approximately 60–70 registered nurses through a team of managers.
• Build high-performing, collaborative teams delivering interdisciplinary transitions-of-care support during the 30-day period after hospital discharge.
• Drive hiring, training, workflow improvement, skill development, and performance management for frontline teams and managers.
• Track productivity, care quality, and clinical outcome KPIs to reduce avoidable admissions and readmissions.
• Coach managers and frontline staff through regular feedback, performance reviews, and continuous improvement activities.
• Provide direct frontline patient care for 10%–20% of the work week and partner with clinical and operational leaders on program strategy.

Requirements

• 5+ years leading high-performing clinical teams in transitional care or care management, ideally across virtual or geographically dispersed teams.
• 3+ years coaching and developing managers or other leaders.
• Active RN license in good standing and/or a BSN or equivalent related degree; a master’s degree is preferred.
• Experience with post-acute medicine, nursing, case management, internal medicine, urgent care, emergency room, hospital medicine, or geriatrics.
• Experience working in interdisciplinary care teams and leading transformational change.
• Ability to work remotely from the United States.
