Transitions of Care Medical Assistant - Member Outreach

Salary: $23.00–$27.00 per hour

What you will do

• Call patients after hospital discharge to assess their condition and confirm they have received necessary assistance.
• Coordinate transportation, durable medical equipment, meals, home health care, and other services with affiliated vendors.
• Schedule post-discharge visits with advanced practice providers, primary care physicians, and specialists.
• Collaborate with patients, providers, caregivers, and the Transitions of Care team to support positive outcomes and prevent readmissions.
• Obtain discharge records from medical providers and facilities and participate in pilot programs.
• Escalate patient needs to the clinical team according to established processes.

Requirements

• Active and clear Medical Assistant certification.
• 2–3 years of experience in healthcare, managed care, home health, and/or community health.
• Ability to work remotely from the United States.

• Full bilingual proficiency in English and Spanish.
