Community Health Worker, Patient Engagement (Pediatric Healthcare)

Location: Remote in Southern New Jersey with frequent travel throughout the area

Salary: $23–$28 per hour, plus an annual bonus incentive

What you will do

• Re-engage patients and caregivers who have not completed onboarding or actively used services.
• Build relationships through direct, in-person community outreach, facility rounding, community events, and telephone engagement.
• Support program re-enrollment, onboarding, mobile app setup, and access to hirify.global services.
• Connect families with primary care providers, pharmacies, social services, behavioral health providers, and other community resources.
• Provide non-clinical care coordination, discharge and follow-up support, and assistance with social determinants of health needs.
• Collaborate with Outreach Specialists, Registered Nurses, Behavioral Health teams, caregivers, families, and facility staff to remove barriers to care.

Requirements

• 3+ years of experience in a healthcare setting, including a patient-facing hospital or facility role.
• Community Health Worker certification required.
• Bachelor’s degree required.
• Valid driver’s license, personal vehicle, and ability to travel frequently required.
• Bilingual Spanish required. Medical Interpreter Certification is preferred.
• Ability to work with interdisciplinary teams, manage multiple technology platforms, navigate ambiguity, think critically, take initiative, and stay organized.
