Senior Backend Engineer II - Patient Experience

Location: Vancouver, British Columbia, Canada. Preference for candidates based in Vancouver, with a flexible hybrid arrangement during the initial onboarding period.

Base salary: CAD 145,000–160,000 per year, depending on experience and geographic location.

What you will do

• Own the patient communications platform from architecture and design through rollout, technical roadmap, and on-call standards.
• Build scalable Python and FastAPI backend services using AWS event-driven services, including EventBridge, SQS/SNS, Lambda, and ECS.
• Deliver reliable patient messaging across email, SMS, and push with idempotency, deduplication, retries, backoff, dead-letter handling, replay, and message-status tracking.
• Implement consent, channel and category preferences, unsubscribe and STOP propagation, quiet hours, and separation of transactional and marketing messages.
• Design safe tooling for one-off and bulk sends, including audience previews, test sends, rate limits, kill switches, and least-privilege access.
• Develop OpenAPI-first patient-facing APIs for web and mobile clients, improve observability and incident response with Datadog, mentor engineers, and guide architectural decisions.

Requirements

• 7+ years of backend engineering experience owning production services, preferably with Python or a comparable backend language.
• Strong experience designing REST APIs, including contracts, versioning, backward compatibility, and error handling.
• Hands-on AWS experience with event-driven systems, modern CI/CD, distributed systems, external APIs, webhooks, retries, rate limits, and partial failure handling.
• Deep knowledge of PostgreSQL and experience with NoSQL databases such as DynamoDB and caching systems such as Redis.
• Experience with authentication and authorization, backend testing, monitoring, observability, on-call duties, and incident response.

Nice to have

• Experience with Iterable or similar customer engagement platforms, SMS providers, push notifications, and email delivery systems.
• Experience with APNs, FCM, Twilio, AWS SES, SPF/DKIM/DMARC, webhook processing, and notification orchestration.
• GraphQL, Backend-for-Frontend patterns, or collaboration with native iOS and Android teams.
• Familiarity with FHIR, Terraform, Docker, GitHub Actions, or highly regulated industries handling sensitive health data.
