Principal Engineer (AI)

Location: Remote across the United States; engineering hub in Orlando, Florida, with occasional visits. Preference for Eastern or Central time to support team overlap.

hirify.global builds an AI operating system for specialty pharmacy that streamlines prior authorizations, patient communication, and coordination across health systems.

What you will do

• Own architecture across multiple product areas and design systems that can scale 10–100x without a rewrite.
• Build safe, production-ready voice and SMS AI agents for patient communication.
• Develop evaluations, guardrails, red-teaming practices, and pharmacist-oversight workflows.
• Design deep Epic EHR integrations, including queues, prior authorization automation, and 340B compliance workflows.
• Define target-state architecture and help migrate the platform from several health systems to hundreds.
• Improve AI engineering practices across model selection, context management, cost, latency, and evaluation methodology while mentoring engineers and leading cross-team projects.

Requirements

• Recent hands-on experience building and shipping serious production systems at Staff+ level.
• Experience making architectural decisions that remain foundational to a company or product.
• Production experience with LLMs, including agents, tool use, evaluations, and guardrails beyond basic SDK integrations.
• Ability to simplify architecture and make pragmatic choices, including structured monoliths and PostgreSQL where appropriate.
• Experience in environments where mistakes are costly, such as healthcare, fintech, fraud, trust and safety, or compliance-heavy systems.
• Must be able to work remotely from the United States; Eastern or Central time is preferred.

Nice to have

• Experience rolling out agentic development practices at team or company level and measuring their impact on output and quality.
• Experience with AI safety or LLM security in production.
• Prior healthcare experience.
