Project Director, Value-Based Care (Healthcare)

Location: College Station, Texas; up to 50% travel to rural healthcare facilities and other locations across Texas. Work may include extended hours and weekends.

What you will do

• Direct value-based care projects helping rural hospitals transition to population health, alternative clinical models, and alternative payment models.
• Approve project plans, define scopes and deliverables, allocate resources, and track timelines, budgets, quality metrics, and outcomes.
• Lead and supervise care coordination teams, including work assignment, performance evaluation, training, and professional development.
• Provide technical assistance to rural hospitals, clinics, healthcare consultants, vendors, public payers, and other partners.
• Monitor performance benchmarks with ACO and CIN partners and direct care coordination interventions to close identified gaps.
• Prepare reports, analyses, presentations, and program materials while representing the department at conferences and stakeholder meetings.

Requirements

• Bachelor’s degree in healthcare administration, public health, or a related field.
• At least 10 years of healthcare operations experience, including project management or project coordination in a healthcare setting.
• Experience with value-based care, population health, or care coordination.
• Knowledge of alternative payment and care models, healthcare regulations, payer contracts, quality metrics, data management, and reporting.
• Ability to manage multidisciplinary teams and complex projects, build stakeholder relationships, provide technical assistance, and communicate effectively.
• Proficiency with electronic medical records, EMR integration and performance data, Microsoft Office, and project management software.

Nice to have

• Master’s degree in healthcare administration, public health, or a related field.
• Experience with Clinically Integrated Networks, Accountable Care Organizations, HEDIS, CMS Star Ratings, MIPS, or HCC coding.
• Experience serving rural or underserved populations and partnering with commercial vendors on population wellness programs.
• Familiarity with telehealth platforms, public speaking, stakeholder engagement, and capacity-building initiatives.
