Care Manager (Behavioral Health)

Location: Remote, but applicants must reside in Florida. Work schedule: Monday-Friday, 8am-5pm Eastern Time.

Salary: $56,200-$101,000 per year.

What you will do

• Assess member needs through phone, digital, in-home, and other site visits, including behavioral health and social determinant needs.
• Develop and monitor personalized care plans for members with high-acuity mental health and substance use disorder needs.
• Coordinate care between members, families, caregivers, providers, specialists, community resources, and payer teams.
• Track treatment side effects, complications, clinical symptoms, services, and member outcomes.
• Educate members and families about treatment options, referrals, healthcare benefits, procedures, and provider instructions.
• Document care management activities and provide feedback to leadership on care quality and cost-effectiveness.

Requirements

• Must reside in Florida and work Eastern Time hours.
• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• Two to four years of related professional experience.
• Active Florida licensure required, such as LCSW, LMSW, LMFT, LMHC, LPC, or RN according to state contract requirements.
• Behavioral health experience and at least one year of professional pediatric care experience.

Nice to have

• Medicaid experience with foster care or adoption programs.
