Ambulatory Care Clinical Pharmacist (Healthcare)

Location: 100% remote within Massachusetts; an active, unrestricted Massachusetts pharmacist license is required at hire and throughout employment.

Salary: $150,000–$162,000 per year

What you will do

• Provide Comprehensive Medication Management with hub-based and virtual clinical care teams.
• Develop medication management action plans with measurable outcomes, monitoring parameters, and follow-up timelines.
• Collaborate with internal and external primary care providers, specialists, nurses, community partners, and other care team members.
• Identify medication therapy opportunities and provide guidance on drug information, dosing, interactions, adverse effects, and member-specific needs.
• Deliver staff education, create medication management materials, and support maintenance of best possible medication lists and post-discharge medication reconciliation.
• Develop medication management pathways and disease-specific protocols toward collaborative practice agreements that enable pharmacist-led assessments, laboratory orders, and medication adjustments.

Requirements

• Active, unrestricted Massachusetts pharmacist license required.
• Completion of both a PGY-1 Pharmacy Practice Residency and a PGY-2 Ambulatory Care Pharmacy Residency, or at least two years of clinical pharmacist experience in an ambulatory care setting.
• Ability to provide direct care to members and caregivers and manage chronic disease and preventative care medication needs.
• Ability to collaborate with multidisciplinary internal and external care teams.
• Medical clearance is required for member-facing work, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, unless an approved medical or religious accommodation applies.
