TL;DR
Senior Backend Engineer (Python/FastAPI) (Medical Imaging): Building and operating a backend for clinical medical imaging workflows with an accent on asynchronous processing, data correctness, and production reliability. Focus on designing event-driven pipelines, enforcing PostgreSQL concurrency and multi-tenant isolation, and integrating resiliently with authentication and GPU inference services.
Location: Taipei City, Taiwan; on-site, full-time

What you will do
- Own the Python/FastAPI backend, including HTTP APIs, background workers, and production behaviour.
- Design and evolve the PostgreSQL data model, migrations, transactions, concurrency controls, indexes, and query performance.
- Build asynchronous, event-driven medical image processing pipelines covering ingestion, de-identification, inference submission, and result handling.
- Integrate authentication, GPU inference, and usage-metering services while maintaining resilience, retry behaviour, timeout handling, and patient-data privacy.
- Define product-to-model and frontend API contracts, lead design reviews, and maintain architecture decision records.
- Support the legacy TypeScript service until its retirement.
Requirements
- 5+ years of professional backend engineering experience, including production ownership of a system designed by you.
- Strong backend fundamentals and willingness to work effectively with Python, FastAPI, and asynchronous code.
- Strong PostgreSQL and SQL skills, including data modelling, transactions, concurrency control, indexing, and migrations.
- Experience with message queues, event-driven architecture, idempotent retries, dead-lettering, timeout handling, and failure-prone external services.
- Experience with static typing, automated testing, code review, CI/CD, Docker, and AI coding assistants.
- Comfortable working in both Chinese and English.
Nice to have
- Google Cloud Platform experience.
- Medical imaging, DICOM, pydicom, or regulated healthcare software experience involving protected health information.
- Multi-tenant SaaS, database-enforced isolation, row-level security, or on-premise product delivery experience.
- Numerical or geometric computing with NumPy, plus familiarity with React and TypeScript.
