Patient Care Coordinator – Virtual Care

Location: Spartanburg, South Carolina, United States; hybrid schedule with work performed from home most of the week and on-site attendance approximately 3 days per week when scheduled.

What you will do

• Communicate with patients through virtual care channels.
• Monitor patient vital signs and lifestyle data.
• Document patient interactions accurately.
• Help patients remain engaged with their care plans.
• Learn and apply Virtual Clinical Monitoring practices.

Requirements

• CCMA or CMA certification, or an LPN license.
• Strong communication and organizational skills.
• Comfort with technology and learning new systems.
• Dependability, reliability, and willingness to learn virtual care skills.
• Reliable laptop and transportation.
• Comfort working as a 1099 Independent Contractor and attending the on-site location approximately 3 days per week when scheduled.
