TL;DR
Quality Care Member Advocate (Healthcare Quality): Connecting members with healthcare services and community resources while identifying and closing care gaps with an accent on member education, home-based assessments, and care coordination. Focus on navigating complex healthcare systems, supporting preventive and chronic care, collaborating with providers on HEDIS and CAHPS measures, and improving outreach outcomes.

Location: Remote-AZ; candidates must be based in Maricopa County. This hybrid role combines approximately 50% remote work with 50% in-person home visits and community-based assessments. Schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m. MST, with flexibility required for member visits. Mileage reimbursement is provided for community travel.

Salary: $27.02–$48.55 per hour.

What you will do

  • Conduct community outreach, home visits, and assessments to identify care gaps, member needs, and social determinants of health.
  • Help members navigate healthcare and social service systems, schedule appointments, understand care plans, and access benefits.
  • Educate members on preventive care, chronic condition management, and available community resources.
  • Collaborate with providers and internal Quality, Care Management, and Provider Relations teams on care coordination and quality improvement.
  • Document member interactions, referrals, and care gap closures in appropriate systems.
  • Monitor outreach effectiveness, support seasonal campaigns, and build relationships with community organizations.

Requirements

  • Bachelor's degree in Social Work, Public Health, Nursing, or a related field, or equivalent experience.
  • At least 2 years of experience in community health or healthcare quality.
  • Experience working with health plan members and navigating community resources.
  • Familiarity with Medicaid, Medicare, and quality measures such as HEDIS.
  • Strong communication and interpersonal skills.
  • Compliance with applicable state and federal regulations and organizational policies.

Nice to have

  • LCSW license preferred.
  • RN state or compact-state licensure preferred.
