What you will do

• Coordinate clinical services and support timely care delivery to patients and families in their homes.
• Triage calls from physicians and clinical staff, follow up on orders, arrange visits, and communicate required changes.
• Provide direct patient care and clinical guidance regarding visit frequency, patient interventions, skills, and homebound status.
• Collaborate with clinical managers, scheduling coordinators, field staff, and multidisciplinary clinicians to maintain appropriate coverage and efficient operations.
• Support staff orientation, preceptorship, competency evaluation, performance reviews, and clinical audits.
• Maintain accurate documentation and ensure compliance with Medicare, HIPAA/HITECH, regulatory, infection-control, and agency requirements.

Requirements

• Active RN license in the state where services are provided; any additional required state license must be obtained within 60 days.
• Three years of community health experience, or at least one year of acute medical/surgical nursing experience combined with two years of home health experience.
• CPR certification upon hire for candidates hired before September 28, 2026, or BLS certification within 30 days for candidates hired after that date.
• Knowledge of Medicare Conditions of Participation, PPS, case management, reimbursement criteria, regulations, and clinical documentation.
• Strong planning, problem-solving, decision-making, leadership, communication, organizational, and interpersonal skills.
• Ability to read, write, and communicate in English. Computer proficiency and the ability to work with a multidisciplinary home health team are required.

Nice to have

• Prior telephone triage experience.
• Experience with word processing, database, and other systems applications.
