Social Worker Hospice Inpatient Part Time Days

Location: 251 East Huron Street, Chicago, Illinois, United States; onsite, part-time day shift

Salary: USD 33.82–43.96 per hour

What you will do

• Complete thorough psychosocial assessments for new patients.
• Screen high-risk social cases using critical thinking.
• Provide psychosocial counseling, interventions, health education, financial counseling, referrals, and discharge or transition planning.
• Collaborate with clinical teams, ancillary departments, case management staff, and social service agencies to develop care plans.
• Connect patients and families with financial assistance, transportation, scheduling, and community resources.
• Participate in case conferences, quality improvement initiatives, and other department activities.

Requirements

• Master's degree.
• Licensed Social Worker or Licensed Clinical Social Worker license in Illinois.
• Zero to two years of experience in a relevant role.
• Ability to work onsite in Chicago during the day shift.
• Successful completion of a background check is required for new hires and certain internal transfers.
