Social Worker Hospice Inpatient Full Time Days

What you will do

• Complete psychosocial assessments for new patients and identify high-risk social cases.
• Provide psychosocial counseling, interventions, health education, financial counseling, referrals, and discharge or transition planning.
• Advocate for patients and families and support continuity of care.
• Collaborate with care teams, case management staff, ancillary departments, and social service agencies to develop care plans.
• Connect patients and families with financial assistance, transportation, scheduling, and community resources.
• Participate in case conferences, quality improvement initiatives, and other department activities.

Requirements

• Master's degree required.
• Illinois Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license required.
• Zero to two years of experience in a relevant role.
• Onsite daytime work in Chicago is required.
• Ability to provide psychosocial counseling, patient advocacy, care coordination, and resource navigation.
