Salary: $115,200–$158,400 per year, plus eligibility for a bonus incentive plan.

What you will do

• Prepare, dispense, and perform quality control checks on home infusion medications and sterile IV admixtures.
• Design and monitor individualized patient care plans, therapeutic drug regimens, laboratory values, and pharmacokinetic outcomes.
• Provide pharmaceutical recommendations, regimen adjustments, counseling, and education to patients, caregivers, nurses, technicians, and physicians.
• Monitor pharmacy operations, inventory, controlled substances, medication errors, adverse drug reactions, recalls, and product incidents.
• Maintain compliance with USP 797/800, DEA, BOP, ACHC, company policies, and applicable pharmacy regulations.
• Participate in continuity of care, quality improvement activities, committees, continuing education, and on-call coverage.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure in the state of practice; Pharm.D. or RPh.
• Working knowledge of sterile IV admixtures, aseptic technique, and USP 797/800 guidelines.
• 1–2 years of clinical pharmacy experience in home care, hospital, or infusion settings.
• Effective written and verbal English communication.
• Ability to lift and push up to 50 pounds and perform extended standing, bending, lifting, and other physical activities.
