Home Health RN, Registered Nurse, Case Manager, Lowell Area

Location: Lowell, Massachusetts and surrounding areas; patient care is provided in clients' homes and the community.

Salary: $84,300.22–$107,481.20 annually based on full-time equivalent.

What you will do

• Assess patients' physical, psychosocial, educational, and care needs in the home setting.
• Develop and update treatment plans in collaboration with patients, families, physicians, interdisciplinary teams, and community resources.
• Provide nursing interventions, medication administration and reconciliation, symptom management, supportive care, and complex home health services.
• Manage an assigned caseload, triage visits and communications, and respond to urgent needs and crisis situations.
• Document patient data, physician orders, OASIS assessments, care activities, and communications in accordance with regulatory and payer requirements.
• Coordinate care conferences, discharge planning, and continuation or termination of services.

Requirements

• Active RN license in the state where services are provided; additional required state licensure must be obtained within 60 days.
• Valid state-issued driver's license.
• At least one year of acute medical/surgical nursing experience.
• Basic Life Support certification; CPR is required upon hire for candidates hired before September 28, 2026, while candidates hired after that date must obtain BLS within 30 days.
• Ability to communicate, read, and write in English.
• Ability to work autonomously in a home care setting, manage time, prioritize schedules, and perform the physical requirements of field nursing, including lifting up to 100 pounds occasionally.

Nice to have

• Bachelor of Science in Nursing.
• Specialty nursing certifications.
• Three years of acute nursing experience, including medical/surgical, oncology, hospice, or home health.
• Experience with IV therapy and complex wound care.
