Government Programs Care Manager III (Healthcare)

Location: Remote within the continental United States, excluding California, New York, Alaska, and Hawaii; required field-based care near Albuquerque, New Mexico, with travel within the assigned territory.

Base pay: $30.45–$54.98 per hour.

What you will do

• Provide telephonic and field-based care coordination for government program members.
• Conduct health screenings, comprehensive health assessments, and care planning for complex and specialty populations.
• Educate members about healthcare programs and support adherence to personal health goals.
• Respond to member inquiries and discuss medical needs with members, physicians, internal staff, vendors, and community resources.
• Complete home or facility visits and support provider and member activities within the assigned territory.
• Analyze medical data and contribute to clinical and population management operations.

Requirements

• RN with at least 2 years of direct clinical care experience, or an eligible unrestricted behavioral health or social work license applicable to the state of operations, with 2 years of clinical practice experience.
• Current, valid, unrestricted licensure in the state of operations or reciprocity; compact RN licensees changing permanent residence must obtain active state licensure within 90 days of hire.
• At least 3 years of wellness or managed care experience presenting clinical issues to members and physicians.
• Knowledge of healthcare and wellness markets, medical data analysis, and Microsoft Word, Excel, PowerPoint, databases, and web-based applications.
• Current unrestricted driver's license, transportation, applicable insurance, and willingness to travel within the assigned territory.
• Must be able to provide required field-based care near Albuquerque, New Mexico, and work from within the continental United States.

Nice to have

• Three years of direct clinical experience, patient education, condition management, or complex and catastrophic case management.
• Bilingual English and Spanish communication skills.
• Case management, training, project management, or nationally recognized healthcare certification.
• Government programs or population management experience.
