Nurse Practitioner (PRN) – In Home Assessments

Location: Fresno, California, United States; on-site. Daily travel is required within a 55-mile radius of the assigned county.

Pay: Approximately $100 per completed assessment plus paid mileage.

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform patient histories, physical examinations, vital-sign checks, medication reviews, and screenings.
• Complete screenings including Hemoglobin A1C, spirometry, KED, DRE, and FIT.
• Educate members on preventive care and chronic disease management.
• Document assessments in the EMR to support value-based care and risk adjustment.
• Work independently in the field while honoring confirmed visits and assignments.

Requirements

• Active, unencumbered Nurse Practitioner license in the applicable state.
• At least 3 years of patient care experience preferred.
• Comfort with EMR systems and independent work.
• In-home assessment or risk-adjustment experience preferred.
• Maintain ongoing monthly availability with a minimum commitment of approximately 30 hours per month.
• Willingness to obtain additional licensure if needed, with support from hirify.global.
