Registered Nurse-Hospital at Home-Part Time Days

Location: Lake Forest, Illinois, with home visits within a defined geographic area

Salary: USD 37–59.20 per hour

What you will do

• Provide hospital-level acute nursing care to patients in their homes as part of the Hospital at Home team.
• Complete comprehensive assessments, nursing diagnoses, care plans, clinical documentation, and ongoing evaluations.
• Coordinate care with physicians, advanced practice providers, paramedics, pharmacists, case managers, social workers, therapists, and other staff.
• Educate patients and families about medications, care plans, discharge instructions, and available resources.
• Respond to changes in patient condition, coordinate escalation or transfer when needed, and support discharge planning.
• Maintain safe, evidence-based practice through medication safety, quality initiatives, technology use, and interdisciplinary handoffs.

Requirements

• BSN required; an ADN is acceptable with a commitment to obtain a BSN within three years of employment.
• Current Illinois Registered Professional Nurse license required.
• American Heart Association Basic Life Support certification required.
• At least one year of recent nursing experience in an acute or emergency setting.
• Strong interpersonal, collaboration, analytical, leadership, and communication skills.
• Valid driver’s license, annual driving abstract, and ability to travel within the designated geographic area required.
