Location: Remote, with required travel to provider offices in and around Christopher, East St. Louis, Edwardsville, Belleville, and Carbondale, Illinois, and Evansville, Indiana. Candidates must be located in the listed U.S. service areas and authorized to work in the U.S. without current or future employment-based visa sponsorship.
Salary: $27.02–$48.55 per hour

What you will do
- Act as a liaison and advocate between members, families, physicians, facilities, and community agencies.
- Conduct face-to-face education in provider offices and telephonic outreach to address preventive care gaps and barriers to care.
- Advise provider practices on HEDIS measures, ICD-10 and CPT coding, and NCQA requirements.
- Assess provider performance data and collaborate with Provider Relations on quality, risk adjustment, operations, claims, and encounters.
- Schedule appointments, arrange transportation, connect members with community resources, and coordinate follow-up services.
- Document member contacts, resolve issues with internal departments, and refer members to case or disease management when appropriate.
Requirements
- Bachelor’s degree in healthcare, public health, nursing, psychology, social work, health administration, or a related field, or equivalent experience; four years of total experience are required.
- At least two years of experience in direct patient care, social work, quality improvement, or health coaching, preferably in managed care.
- Preferred credentials include LPN, LMSW, C-SW, LSW, or RN.
- Must be authorized to work in the U.S. without employment-based visa sponsorship now or in the future.
- Fluency in English and Spanish is highly preferred.
- Regular travel to provider offices is required.
