Senior LTSS Service Care Manager RN

Location: Hybrid role based in McAllen, with in-person assessments across the Harlingen, Mercedes, and Weslaco service area. Home and other site visits may be required once a month or more.

Salary: $36.21–$65.09 per hour

What you will do

• Assess complex, high-risk, and high-acuity members and recommend appropriate care and service plans.
• Develop, monitor, and update personalized long-term care plans in collaboration with members, caregivers, providers, specialists, and community resources.
• Coordinate medical, supportive, and community services, including service authorization for meals, transportation, housing, employment, foster care, and activities of daily living.
• Monitor member status, clinical symptoms, complications, care gaps, waiver eligibility, and potential need for a higher level of care.
• Educate members and families on disease processes, care options, referrals, provider instructions, and healthcare benefits.
• Document care management activities, support regulatory compliance, advocate for members, and contribute to quality and cost-effective service delivery.

Requirements

• Graduate degree from an accredited school of nursing or a bachelor's degree with 4–6 years of related experience.
• Bachelor's degree in Nursing preferred.
• Current RN state licensure and/or compact state licensure, or current state licensure as a Nurse Practitioner.
• Resource Utilization Group certification must be obtained within 90 days of hire.
• Ability to perform in-person assessments and occasional home or site visits within the assigned service area.
