Wellness Coordinator - Registered Nurse (Part-Time)

Location: On-site in Middletown and Westerly, Rhode Island, USA; travel between both locations is required.

Salary: $32–$37 per hour plus a bonus structure, depending on licensure and certifications.

Behavioral Health Group is a network of Joint Commission-accredited treatment centers providing opioid addiction treatment and support for people with substance use disorders.

What you will do

• Lead and manage the Health Home team to provide comprehensive support and care for patients.
• Identify opioid treatment program patients, conduct outreach, assess needs, and develop individualized care plans.
• Assign team roles, establish care protocols, monitor treatment progress, and coordinate Health Home services.
• Supervise case managers and facilitate team meetings.
• Manage patient engagement, care coordination, health promotion, transitions of care, and community referrals.
• Implement regulatory compliance and quality improvement activities to enhance patient care.

Requirements

• Master’s degree in Social Services, Psychology, Social Work, or Criminology from an accredited institution, or qualification as a Registered Nurse.
• Ability to work on-site and travel between the Middletown and Westerly locations.
• Availability for 5:00 a.m. to 1:00 p.m. shifts three days per week.
• Relevant licensure or certifications may affect compensation.
